The following documents are required to accompany forms outlined above and complete enrollment:

Original Birth Certificate school office is to make a copy from the original
Student's Immunization Record or physician may fax to school
(2) proofs of in-district residency:
- (1) a copy of resident guardian's electric, gas, or water utility bill (at least 30 days of residential service must be documented, service address must match that of student as well as the lessee or property owner listed on proof #2)
  AND
- (2) a copy of resident guardian's rental/lease agreement OR real estate tax statement if purchased/contract if contracted to be purchased OR if residence is not rented or owned by resident guardian, an affidavit may be requested from the school (see note below). The student's residence must match that of the guardian.

*Phone, trash, and internet bills are not considered proof of residency as they do not indicate usage of a utility on/residence of the property.
**A valid affidavit must be notarized - completed by both the property owner and the resident guardian in the presence of a notary.
