Employee Health Insurance

The Blair Oaks R-II School District is a member of the Ozarks School Benefits Association.  This is a self-funded consortium that has been providing employee benefits solutions for public school districts since 2009.

Acrisure has been the employee benefits administrative broker for OSBA since the founding of the program. Their agency and staff manage the program and assist you with your benefit needs.

The District provides health insurance for all full time employees– $644/month (base PPO Plan rate) for each employee. See below for details.  Employees may add coverage for their family members at their own expense.

Any and all insurances are OPTIONAL, with the exception of the board-paid life insurance policy.  Insurances must be elected within 30 days of hire, or during an open enrollment period.

Health Insurance – Anthem

  • Begins on page 7 in the Benefits Guide

  • PPO (Preferred Provider Organization) Plans

    • There are 4 PPO plan options to choose from.

    • The District pays for 100% of the employee’s monthly premiums for the base PPO plan ($644/month).

      • This is the 2500/3000 Choice PPO plan.

    • If you elect a lower deductible PPO plan, you are responsible for the remainder.

  • HSA (Health Savings Account) Plans

    • An HSA is a bank account you deposit pre-tax money into to be used to pay for current or future qualified health expenses for you, your spouse, and/or your eligible dependents.

    • Money never goes away (NOT “use it or lose it”)

    • You will receive a debit card.

    • There are 4 HSA Plan options to choose from.

    • The district pays $644/month toward your medical insurance.  All HSA plan premiums are less than this amount.  The difference is placed into an HSA for your use.

      • Example: If you choose the 6000/6500 Choice HSA, the employee monthly premium is $514.  The district will pay for this completely, and also contribute $130 to your HSA, totaling the $644.

    • You may also contribute any amount you would like toward your HSA.

Board-Paid Life Insurance – The Hartford

The District pays for a small Life Insurance/AD&D policy for each employee.  This is the only insurance policy that is mandatory.  We will need beneficiary information to enroll you.

  • $15,000 Basic Group Term Life and Accidental Death and Dismemberment coverage.

  • Dependents cannot be added to this policy, but can be added to other voluntary life insurance options listed below.

  • Flexible Spending Account (FSA)

    • An FSA is an employee benefit that allows you to set aside pre-tax money to pay for qualified health care and dependent care expenses.

    • Must be used by the end of the calendar year (“use it or lose it”)
