Enrollment Forms

 If you are new to the Blair Oaks R-II School District and will have a student entering the elementary school, please click on the link below to print the enrollment forms required. If you have any questions, please call the Elementary office at (573) 634-2808. 

New students must provide all of the following:
1. student's birth certificate
2. immunization record
3. (2) proofs of residency [(1) copy of electric or water bill AND (2) rental/lease agreement or real estate tax statement].
